  • replace a food license that has been lost/stolen
  • request reciprocity (or get a state license with a valid FSSMC license from another state or Chicago)
  • pay for a license if you lost or never paid the IDPH invoice for the public health license (the state automatically invoices upon successful completion of the exam, although it may take 4-6 weeks after test completion)

  • obtain a food sanitation (FSSMC) license after passing the licensing exam (Chicago does not automatically invoice for the FSSMC license once the course/test have been successfully completed.)
  • replace a food license that has been lost/stolen
  • request reciprocity (or get a city license with a valid FSSMC license from Illinois or another state or Chicago)
